Documents issued by FL Circuit Court, FL Department of Health’s Office of Vital Records, or the State Corporation Commission.

Acceptable Identification Requirement

Valid ID

A notary is required to exercise a high degree of care in determining the identity of any person whose identity is the subject of a notarial act. At least one of the following nine unexpired documents is required for the principal and any witnesses (if applicable):

  1. FL driver’s license or ID card issued by the Dept of Highway Safety and Motor Vehicles
  2. United States Passport/Passport Card
  3. Foreign passport if stamped by the U.S. Citizenship and Immigration Services
  4. Driver’s license or ID issued by another state
  5. Driver’s license officially issued by Mexico or Canada
  6. United States military identification card
  7. Inmate ID issued on or after Jan 1, 1991, by the FL Dept of Corrections or Federal Bureau of Prisons
  8. Sworn, written statement from law enforcement officer, explaining an inmate’s ID was confiscated upon incarceration and that the person named in the document is the person whose signature is to be notarized
  9. Permanent resident card or “green card” issued by the U.S. Citizenship and Immigration Services
  10. Veteran’s health ID card issued by the U.S. Dept of Veterans Affairs

Credible Identifying Witnesses if Signer Does Not Have Proper Identification

When a document signer is not personally known to the Notary and is unable to present an acceptable ID, the signer may be identified through the oath (or affirmation) of two (2) credible identifying witnesses. Each witness must produce acceptable ID.
